Dear Parents Enclosed are the coach’s appraisal surveys for your team. One sheet is for your comments as parent and the other sheet is for your child to fill out. After you finish the survey, please staple or tape the papers shut at the top of the fold and place back in the enclosed envelope to mail back to: TUCSC Division Coordinator c/o Wendy McLaughlin 200 Versailles Court Tracy, CA 95376 The results of this survey are for the benefit of your coach (and assistant coach) to help him/her identify issues or improve coaching style. Comments can be continued on the back of the page and may address either the coach or assistant coach (please be specific). I encourage every player and one parent from each family to complete the form and return it. These are intended to be confidential. Names are optional. Please mail the forms by January 25th, 2008.
As always, if there are any issues or concerns regarding TUCSC, competitive soccer, or coaching that you want to speak about personally, please feel free to contact your appropriate Division Coordinator.
